A question you wish you could ask a nurse. A care plan you want to understand. A loved one who needs a little more support. WellNES offers personalized nursing guidance by phone or video, alongside in-person care on the Central Coast.

You don’t need a home visit to get started. We can help you prepare for appointments, make sense of care instructions, organize your health information, and feel more confident about your next step.

Expert nursing support. Centered on you. Personalized, RN-led guidance to help you understand your health, navigate care, and feel more confident about your next steps. From answering questions and organizing health information to preparing for surgery or supporting a loved one, we offer practical education, nursing assessment, and care coordination—in your home or virtually. You don’t need an existing care plan to get started.

WellNES Health Passport

Your health information, organized in one place. A nurse helps you build a practical reference for appointments, a new provider, or a transition in care.

Best for: Adults who want their records organized, people seeing several specialists, and families helping a loved one manage care.

Essential — $295: One consultation of up to 60 minutes; review of the information you provide; a medication, supplement, and allergy list; health and surgical history; provider, pharmacy, insurance, and emergency contacts; and a record of whether you have an advance directive or POLST. We organize existing documents; we do not create medical orders or provide legal advice. You receive an organized printed binder, a concise appointment/hospital summary, and two wallet cards with essential contact and allergy information. Includes one correction or update within 30 days.

Complete — $495: Everything in Essential, with a consultation of up to 90 minutes, organization of relevant records you provide or authorize us to obtain, a more detailed review of medication lists for discrepancies to discuss with your prescriber or pharmacist, recent hospitalization and surgery summaries, a copy for one authorized family member or care partner, one follow-up of up to 30 minutes, and corrections or updates for 90 days.

Virtual or in-home: Meet by phone/video to review your information, or meet at home to organize documents together. We arrange how you receive your printed materials. The Passport is a reference based on available information; it does not replace your providers’ medical records.

Wellness Guidance & Planning

A practical starting point when you want direction. Talk through your questions, current routines, and priorities with a nurse, then leave with a focused nursing support plan based on your goals and existing care recommendations.

Best for: Adults who feel overwhelmed by health information, want help preparing for a routine appointment, or need support turning existing care recommendations into manageable next steps.

Includes: A review of your goals and concerns, nursing education relevant to those concerns, help identifying questions for your healthcare team, and a written action list with agreed priorities. Follow-up check-ins can be arranged separately.

Virtual or in-home: Phone/video sessions focus on discussion, education, and planning. An in-home session can also consider how your everyday environment affects your routine. This service does not include diagnosis, prescribing, or an ongoing on-call arrangement.

Surgical Planning & Post-Surgical Support

Prepare for surgery and make sense of recovery. We help you work through the practical details before a procedure and understand the plan your surgical team gives you afterward.

Best for: People preparing for an elective procedure, recovering at home, living alone, or relying on a family member who would like guidance.

Includes: Review of your surgical team’s instructions, a preparation or recovery checklist, help organizing medications and follow-up appointments, caregiver education, and questions or concerns to bring back to your surgical team. We help you understand when their instructions call for contacting them.

Virtual or in-home: Phone/video support covers preparation, instruction review, and recovery check-ins. In-home visits can include personalized, hands-on nursing support tailored to your recovery needs. Concerns about complications are communicated to your surgical team or referred for appropriate medical evaluation. Hands-on care is separately assessed and quoted; it cannot be delivered virtually.

Medication Support

A clearer, more manageable medication routine. Bring your bottles, lists, and questions so we can help you understand and organize the instructions you already have.

Best for: People taking several medications, adjusting to a recently changed regimen, or helping a loved one keep track of prescriptions and supplements.

Includes: An organized list of reported medications and supplements, comparison of available lists and instructions, a schedule based on existing directions, education about following that routine, and a list of discrepancies or questions for your pharmacist or prescriber.

Virtual or in-home: Review labels and lists by video, or discuss a prepared list by phone. In-home support allows us to review the physical supplies and discuss storage and organization. We do not start, stop, or adjust prescriptions or resolve conflicting orders independently. Questions go to your prescriber or pharmacist. If you need hands-on medication support, we’ll discuss your needs and available visit options.

Senior & Family Support

Support for an older adult—and the people supporting them. Create a clearer picture of day-to-day needs and a practical plan for staying connected.

Best for: Older adults wanting regular guidance, family caregivers feeling stretched, and adult children helping a parent from a distance.

Includes: A conversation about current concerns and routines, a review of care and appointment needs, caregiver education, an agreed list of next steps, and communication with a designated family member when the client authorizes it. Recurring check-ins can be arranged with a defined schedule.

Virtual or in-home: Phone/video sessions can include the client and authorized family members in different locations. Home visits allow additional observation of the living environment and daily routines. This is nursing support, not continuous supervision or emergency monitoring.

Hospital-to-Home Transition

Turn discharge paperwork into a workable plan. Get help understanding what needs to happen after a hospital stay and who to contact about outstanding questions.

Best for: Someone recently discharged, a person with several follow-up instructions, or a caregiver taking on new responsibilities at home.

Includes: Review of discharge instructions and available medication lists, a follow-up appointment checklist, identification of supply or support needs, caregiver education, and an organized list of questions for the discharging team or regular providers. Coordination with providers is arranged with your authorization.

Virtual or in-home: Phone/video support focuses on reviewing the plan, coordinating next steps, and answering nursing questions. An in-home visit can also help identify practical needs and provide personalized nursing support as you settle back into your routine. Unclear or conflicting instructions are referred back to the treating team. Equipment, transportation, and hands-on care are not automatically included.

Health Navigation & Advocacy

A nurse in your corner when the system feels complicated. Organize the information, questions, and follow-through around a specific healthcare concern.

Best for: People navigating multiple providers, preparing for an important appointment, seeking clarity after a visit, or supporting a loved one through a complex care situation.

Includes: Review of your main concern and relevant information, an appointment question list, help understanding information provided by your care team, a written next-step summary, and agreed coordination with providers or family members when authorized.

Virtual or in-home: Meet by phone/video or at home to review information together. Participation in a provider appointment can be discussed in advance and depends on consent and availability. We support informed conversations; medical decisions remain with you and your treating clinicians.

Apart from the Health Passport packages above, services are quoted individually. Before you book, we’ll confirm the session length, virtual or in-home format, included tasks, written follow-up, any provider coordination, and the price. Additional visits, ongoing messaging, travel, and hands-on care are included only when specified in your plan.

In-person visits are available on the Central Coast, subject to scheduling and nursing assessment. These services do not replace emergency care.

IV Therapy / Coming Soon

IV therapy is coming to WellNES Cayucos on the Central Coast. Our planned service will pair medical evaluation and treatment orders from an authorized prescriber with RN-administered care, under our medical director’s clinical oversight.

IV therapy delivers fluid and selected nutrients directly into the bloodstream. An authorized prescriber will determine whether treatment is appropriate and order the fluids and any nutrients. Here are the goals you can discuss with the clinical team as our service takes shape.

01 / HYDRATION

Replenish lost fluids.

When you’re dehydrated, replacing fluid helps restore your body’s fluid balance. IV hydration may be appropriate when drinking fluids isn’t enough or isn’t well tolerated. For everyday hydration, oral fluids are usually the first step.

02 / NUTRIENT SUPPORT

Address a specific need.

Targeted nutrients may be useful for an identified deficiency or when absorption is a concern. An authorized prescriber evaluates the need for supplementation and determines whether an IV is appropriate compared with oral options.

03 / ENERGY & RECOVERY QUESTIONS

Start with the why.

Feeling depleted after travel, a busy stretch, or activity? Low energy can have many causes. Your nurse can gather your history, assess nursing concerns, and communicate findings. An authorized prescriber evaluates possible medical causes and determines whether IV treatment or another type of care is appropriate.

Your experience will begin with a personalized medical consultation to discuss your goals and whether IV therapy is a good fit for you. If treatment is appropriate, you can expect attentive, one-on-one nursing care, time to ask questions, and support throughout your visit. Care is personalized to your medical history and individual needs. Your prescribing clinician will discuss potential benefits, risks, and alternatives before treatment. Individual results vary.
